My Action Plan
A "My Action Plan" is a form designed to help students identify and solve problems. The student will be asked to fill out a "My Action Plan" form, and then we will go over the plan together. Finally, the student will be asked to sign the form as a commitment in taking the responsibility to alter his or her behavior.
The form consists of the following questions:
1. What is the problem?
This question helps students to isolate and identify the problem.
2. What is causing the problem? (Please list all factors.)
This question helps students to determine the factors causing the problem.
3. What plan will you use to solve the problem?
This question helps students to write a plan of action to solve the problem or to prevent the problem from happening in the future.
